How to submit an opinion request
The CEUMa opinion requests should be done by filling in the Opinion Request Form..
The request should include the following documents, when applicable:
- Information to the participant and Informed consent documents for the participation in research studies, following University of Madeira’s template or other created by an accredited body.
- Data collection tools to be used (scales, questionnaires, observation interview scripts and guides, among others).
- Declaration signed by the research advisor, in case the primary researcher is a student.
- Assent given by the Data Protection Committee of University of Madeira.
- Copy of the notification given to the competent regional, national, European or international authorities, when applicable, together with their respective assents.
- Statement of conflict of interests (if applicable).
- Assent or approval given by the responsible people of the entity(ies) where the study or research project will take place, in case it occurs outside UMa;
- The researcher’s Curriculum Vitae.
